You can download
an application today. All applications received prior
to the 20th of the month will be eligible the first of the
following month. Applications received after that date will
be effective the first of the next month. |

Can I choose my own dentist and
optometrist? |

Yes, you may choose
any provider that is currently participating with Golden
West and that is accepting new members on your plan of choice.

Can I change providers? |

You can change providers by notifying our office prior to the 20th of any given month to be eligible the first of the following month. |

Can anyone join? |

All Students, Graduates and Alumni
that are California residents and their dependents are eligible
to join. |
